The States We Serve

When we opened, Gentle Health served two states: Arkansas and Kentucky. Today we serve 40 states and the District of Columbia.

The mechanism is worth explaining, because it says something about how the practice works. Gentle Health is not a network of contracted prescribers. Every patient, in every state we serve, is evaluated and treated by the same physician: James Simmons, MD. Dr. Simmons was approved through the Interstate Medical Licensure Compact, an agreement among participating state medical boards that allows a physician in good standing to obtain licenses in member states through a single verified application. His Arkansas and Kentucky licenses were the beginning; the Compact made the rest possible.

One physician, individually licensed in each state where we practice. That model does not scale the way a roster of prescribers does, and that is the point.

Our state pages provide Dr. Simmons’s license number and the relevant medical board lookup. Board lookups are free and public, and a practice that will not tell you where to find its physician’s license is telling you something.

What patients in every state get

The same practice, unchanged from one state to the next:

  • One physician, licensed in your state. Every prescription at Gentle Health is written by James Simmons, MD. Not a network, not a rotating panel. He holds an individual license in each state we serve, verifiable through that state’s medical board.
  • Published dose-tier pricing. Compounded oral semaglutide starts at $112 per month, compounded injectable semaglutide at $135, and compounded tirzepatide at $169. Prices are published at every dose tier, month to month, cash-pay, with no membership fee, no insurance requirement, and no contract balance if you stop. Under our terms, you can cancel an order and the subscription any time before the order is sent to the pharmacy, and the subscription 30 days after the order date once it has been sent; the remainder of the current billing month is not refunded.
  • A disclosed process. You complete a medical intake, Dr. Simmons reviews it personally (with a video visit first in the states that require one), and if treatment is appropriate for you, a licensed compounding pharmacy fills the prescription and ships it to your door. If treatment is not appropriate, we tell you that instead. Not everyone who applies is approved, and we consider that a feature of the model rather than a flaw.

Why the list grows the way it does

We started in the two states where Dr. Simmons was first licensed, and we said from the beginning that we would only operate where he personally holds a license. The Interstate Medical Licensure Compact makes that scalable without changing the model: one accountable physician, individually licensed in each state, rather than an “available nationwide” claim that blurs who is actually responsible for your care.

The list grows at the speed of licensure. That is slower than a marketing department would like, and exactly as fast as we think it should be.

Pricing depends on the dose tier Dr. Simmons prescribes. Current monthly prices:

Medication, the initial review, video visits and messaging, and overnight refrigerated shipping are all included. There is no separate membership fee, and HSA and FSA cards are accepted. The monthly price changes only if your prescribed dose moves into a different tier.

Common questions

Is the price the same in every state? Yes. Starting tiers are $112, $135 or $169 per month depending on the medication, and higher prescribed dose tiers have higher published prices, identically in every state we serve. That is the whole bill.

Who will prescribe my medication? James Simmons, MD, the same physician in all 41 jurisdictions. The state pages linked above provide license details and board-verification links.

My state is not on the list. When will you get there? More states are in progress. Each one arrives when licensure is complete, and this page is updated when it does.

How do I know the list is current? This page carries a last-updated date at the top and is revised whenever a state is added. It is the single place we publish our current footprint.

How do I start? From any state on the list, complete the medical intake on this site. Dr. Simmons reviews every intake personally, and treatment proceeds only if it is medically appropriate for you.
