What North Dakota rules require
North Dakota requires an evaluation equivalent to an in-person examination, so your doctor may message you with questions or ask you to book a video visit.
$28 a month for your doctor. You pay LillyDirect for the medicine, at its self-pay price for your dose unless it takes your insurance.

What each price is made of
Gentle Health
Your doctor’s care, $28 a month: reviews, dose decisions, messaging and video visits. Plus LillyDirect’s self-pay price for Foundayo, which you pay LillyDirect.
Ro
The maker’s price for Foundayo, plus Ro’s membership: $39 the first month, then $149 a month.
No, we don’t bill insurance or file prior authorizations. You pay $28 a month for the doctor, by card or with an HSA or FSA card. LillyDirect takes commercial insurance for Foundayo, but a plan that needs a prior authorization won’t cover it through us; otherwise you pay its self-pay price.
A doctor can check whether Foundayo is safe for you and whether you need it in the first place.
Complete the intake and I’ll review your history. Message me with any questions, or book a video visit if you’d like one. The intake and my first review are free.

How this was studied · ATTAIN-1 trial · 3,127 people · 72 weeks · vs placebo
Who took part
Adults with obesity (BMI 30 or more), or with overweight (BMI 27 or more) plus a weight-related condition such as high blood pressure. People with diabetes were excluded.
How many people
723 on 5.5 mg, 725 on 9 mg, 730 on 17.2 mg and 949 on placebo
How long
72 weeks, including up to 20 weeks of stepping up the dose
What they got
Daily pills of an earlier form of orforglipron, the medicine in Foundayo, at doses equal to Foundayo 5.5, 9 or 17.2 mg, starting at the lowest dose and rising every 4 weeks. Everyone had counseling toward a reduced-calorie diet and at least 150 minutes of activity a week.
Compared with
Dummy pills with no medicine (placebo), plus the same counseling. Nobody knew which they had.
What was measured
Average percent change in body weight after 72 weeks, the trial’s main result (primary endpoint).
Who was counted
Everyone randomized, as the Foundayo label reports it; missing week-72 weights were estimated (imputed). The paper reports 7.5%, 8.4% and 11.2% for the three doses.

Who took part
Adults with type 2 diabetes and a BMI of 27 or more, with an HbA1c of 7% to 10%, managed by diet and exercise or by diabetes pills other than DPP-4 inhibitors or GLP-1 pills. People on insulin or other injected diabetes medicines were excluded.

Nobody knows from a direct test: no published trial has compared them. In separate trials of people without diabetes, average weight loss was smaller on Foundayo than on either shot.

Possibly. On the top dose, 17.2 mg, 35% of people had nausea at some point; stomach side effects were mostly mild or moderate and came mostly while the dose was going up.

Clinical reviewSome other medicinessuch as clarithromycin, ritonavir, carbamazepine or simvastatin: the label says to avoid some with Foundayo and caps a dose with others, so your doctor checks
Clinical reviewOn the birth-control pillit may work less well: switch to birth control that isn’t a pill, or add condoms, for 30 days after starting and after each dose increase
Clinical reviewOther health conditionskidney, gallbladder or thyroid disease, a history of an eating disorder, or severe mental illness: your doctor reviews them with you before prescribing
Possibly: it slows stomach emptying, which may make the pill work less well. For 30 days after starting and after each dose increase, use birth control that isn’t a pill, or add condoms.
1Why: Foundayo slows how fast food and pills leave your stomach, most after the first dose, so less of the pill may be absorbed.
2What to do: for 30 days after starting and after each dose increase, switch to birth control that isn’t a pill, or keep the pill and add condoms.
3Not affected: birth control that isn’t a pill, such as an implant, shot, patch, ring or IUD.
4Not tested: no trial has measured Foundayo’s effect on the pill; the label advises this as a precaution.
A few, such as the antibiotic clarithromycin, the HIV medicine ritonavir, the seizure medicine carbamazepine and the cholesterol medicine simvastatin. Your doctor checks everything you take.
1Medicines that slow the liver enzyme that clears Foundayo (CYP3A4), such as clarithromycin or ritonavir, raise its level in your blood: with some, Foundayo goes no higher than 9 mg a day; others, like ritonavir, shouldn’t be taken with it.
2Medicines that speed that enzyme up, such as carbamazepine, lower its level, so Foundayo may not work: the label says to avoid the strong ones, and with milder ones your doctor watches whether it’s working.
3Simvastatin: Foundayo raises its level, so no more than 20 mg a day.
4Insulin or a sulfonylurea: blood sugar may drop too low, so your doctor may lower their dose.

How this was studied · ATTAIN-MAINTAIN trial · 205 people · 52 weeks · vs placebo
Who took part
Adults without diabetes who had just finished 72 weeks of Zepbound shots (10 or 15 mg) in another trial, SURMOUNT-5, and had lost at least 5% of their weight on them.
How many people
125 on orforglipron and 80 on placebo
How long
52 weeks, starting ideally within 2 weeks of the last shot
What they got
Daily orforglipron capsules, an earlier form of the Foundayo tablet, starting straight at 12 mg (equal to Foundayo 9 mg) and rising every 4 weeks to 36 mg (17.2 mg), or 24 mg (14.5 mg) for people who couldn’t take more. The Foundayo label instead starts everyone at 0.8 mg. Everyone had diet and activity counseling.
Compared with
Dummy pills with no medicine (placebo), plus the same counseling. Nobody knew which they had.
What was measured
The share of the weight lost on the shots still off after 52 weeks, for everyone: a key secondary result. The main result (primary endpoint), in people whose weight had leveled off on the shots, was 74.7% on orforglipron and 49.2% on placebo.
On placebo
From week 24, people on placebo who had regained half or more of the weight they had lost could take orforglipron; the analysis gave them no credit for it. Only 31.3% of the placebo group finished the year without any weight-loss medicine.
What it did not test
Staying on Zepbound: no group kept taking it, so the trial can’t say whether switching keeps as much weight off as staying.

On placebo
From week 24, people on placebo who had regained half or more of the weight they had lost could take orforglipron; the analysis gave them no credit for it. Only 18.2% of the placebo group finished the year without any weight-loss medicine.

Staying on Wegovy: no group kept taking it, so the trial can’t say whether switching keeps as much weight off as staying.
A possible thyroid cancer risk. Other GLP-1 medicines caused thyroid tumors in rats and mice, where orforglipron doesn’t act; nobody knows yet whether it causes them in people.
1Do not take it with a personal or family history of medullary thyroid cancer, or MEN2.
2Tell your doctor about a neck lump, lasting hoarseness, trouble swallowing, or shortness of breath.
A lab-made small molecule, not a peptide, that switches on the same receptor as the gut hormone GLP-1. Semaglutide and tirzepatide each weigh about five times as much.
