LillyDirect is Eli Lilly’s own service for buying its medicines straight from the maker. For self-pay patients it works in four steps: 1. Your prescriber sends the prescription to LillyDirect Pharmacy. 2. A partner texts you to confirm it. 3. Once it is verified, the dispensing pharmacy sends you a checkout link by text or email. 4. You pay online, and the medicine ships free, usually within one to four days of checkout.
As of October 19, 2026, Zepbound costs $299 to $449 per four-week supply and Foundayo $149 to $299 a month, depending on the dose (LillyDirect Zepbound, LillyDirect Foundayo).
Here is what happens at each step, what it costs, and the fine print that changes the price.
Who fills the prescription
The prescription goes to LillyDirect Pharmacy in Indianapolis, but a partner pharmacy fills it. In LillyDirect’s words, it “currently works with four dispensing pharmacies: Amazon Pharmacy, Fuze Health, Gifthealth, and Walmart Pharmacy,” and which one fills yours “depends on your medicine, your chosen delivery method, and your preferred payment method” (LillyDirect). Gifthealth handles self-pay orders. LillyDirect says the Zepbound KwikPen can also be picked up at Walmart Pharmacy instead of delivered.
Step by step
- Your prescriber sends the prescription. It goes to LillyDirect Pharmacy, electronically or by phone or fax. LillyDirect asks that it include “your current mobile phone number and an email address” (LillyDirect FAQ).
- A confirmation text. Within one business day, LillyDirect’s partner Prescryptive texts you to check that the prescription is correct.
- Verification. LillyDirect says to “allow a minimum of 3–5 business days for us to verify your prescription information.”
- Checkout. Once everything is confirmed, the dispensing pharmacy sends you a checkout link by text or email. You can pay with any major credit card or an FSA or HSA card, and with cash if you pick up in person.
- Delivery. “Home delivery through LillyDirect is completely free,” and “most prescriptions are processed and shipped within 1–4 days of checkout.” Refrigerated medicines may not ship close to weekends or national holidays, so the medicine stays cold on the way.
From prescription to your door, that is often a week or more. Plan your first order with that in mind.

|---|---|---|
| 2.5 mg | $299 | $299 |

Prices from Zepbound’s LillyDirect terms, as of October 19, 2026.
Needles are a separate purchase: “a new pen needle is required for each weekly injection” with the KwikPen, and “a new needle and syringe” with the vial. The single-dose autoinjector pen is not sold self-pay on LillyDirect; it goes through insurance. What a full year costs, and how the vial and KwikPen differ, is in how much is Zepbound without insurance?. Our Zepbound care page explains Gentle Health’s prescribing service separately from LillyDirect’s medicine price.

The 45-day rule
The lower prices at Zepbound’s higher doses, and at Foundayo’s two highest doses, depend on refilling on time. You “must complete your refill purchase… within 45 days of the delivery/received date of your previous” prescription. Miss the window and the regular price applies to that fill. With a 28-day Zepbound supply, the window runs 17 days past the supply itself.
Refills and dose changes
Your pharmacy reaches out within about 23 days to start the next refill. If your prescription has no refills left, the pharmacy works with your doctor to request a renewal. LillyDirect fills prescriptions exactly as written, so “any dose changes must come from your provider.”
Insurance, Medicare and eligibility
- Using insurance for the KwikPen. LillyDirect accepts select insurance for this device and checks coverage. Gentle Health does not bill insurance or submit prior authorizations; if your plan requires one, we cannot provide that step. Ask LillyDirect and your plan what your route and copay would be. The prices in the tables above are self-pay prices, not insured copays.
- Paying cash with insurance. You can choose self-pay even if you have insurance, but you agree not to seek reimbursement from your insurer or any other third-party payer.
- Savings cards. Lilly’s self-pay savings cards require you to be 18 or older and a resident of the United States or Puerto Rico. Its $25 cards, for people whose commercial insurance covers the medicine, exclude Medicare, Medicaid and other government programs.
- Medicare. Separately, the temporary Medicare GLP-1 Bridge gives eligible Part D members the Zepbound KwikPen or Foundayo for weight management at $50 a month through December 2027 (LillyDirect Medicare). That runs through Medicare, not the self-pay prices above.
Where Gentle Health fits
LillyDirect sells the medicine, but you still need a prescriber. Our doctor prescribes Zepbound (single-dose vials or the KwikPen) and Foundayo in all 41 jurisdictions where our physician is licensed.
You complete an online intake, and the doctor reviews it in writing or by video visit, depending on your state’s rules. If the medicine is right for you, the doctor sends the prescription to LillyDirect. Watch your texts and email for LillyDirect’s checkout link.
You pay us $28/month for the doctor’s care, including messaging and video visits. You pay LillyDirect for the medicine at the prices above. You pay us only once the doctor writes the prescription, there is no minimum, and you can cancel any time. Gentle Health is not affiliated with Eli Lilly. How our care fee compares with other online providers is on our comparison page.
Frequently asked questions
Do I need a prescription to buy from LillyDirect? Yes. LillyDirect fills prescriptions from a licensed prescriber; it does not prescribe. Your doctor, or a telehealth prescriber such as Gentle Health, sends the prescription to LillyDirect Pharmacy.
How long does a LillyDirect order take? Allow at least 3 to 5 business days for LillyDirect to verify the prescription. After checkout, most orders ship within 1 to 4 days, and home delivery is free.
Can I use my HSA or FSA at LillyDirect? Yes. LillyDirect’s dispensing pharmacies accept FSA and HSA cards, as well as major credit cards.
Can I use insurance for the Zepbound KwikPen through LillyDirect? LillyDirect accepts select insurance for the KwikPen and checks coverage, but Gentle Health does not submit prior authorizations. If your plan needs one, ask your plan or LillyDirect about another prescriber route. The prices shown here are for self-pay.
What happens if I refill Zepbound late? At 7.5 mg and above, the $449 price requires completing your refill within 45 days of your last delivery. Later than that, the fill costs $499 at 7.5 mg or $699 at 10 mg to 15 mg.
Can I buy from LillyDirect if I’m on Medicare? You can pay self-pay if you agree not to seek reimbursement, but Lilly’s savings cards exclude government programs. Eligible Part D members may get the Zepbound KwikPen or Foundayo for $50 a month through the Medicare GLP-1 Bridge.
